Money-Back Guarantee Terms & Conditions: NiaHealth offers a money-back guarantee for new Core members only. This guarantee applies solely to Core membership purchases and is not applicable to any other NiaHealth memberships or products. Limit one refund per person. To be eligible, the member must complete the full Core experience, including required onboarding, blood draw, and clinician consult. Following completion of the full Core experience, the member must submit a refund request within 7 calendar days; requests submitted outside this window are not eligible. Prior to processing any refund, the member must complete a 30-minute feedback call, which must be conducted in good faith. Refunds are limited to the Core membership fees actually paid by the member, up to a maximum of $599; any promotional discounts or credits applied at the time of purchase will be reflected in the refund amount, and no refund will exceed the amount the member originally paid. This guarantee does not apply to renewals, transfers, gifted memberships, or corporate plans. NiaHealth reserves the right to end this offer at any time, modify these terms, or decline refund requests in cases of suspected abuse or fraud.

Promotional Terms & Conditions: Receive$100 off when you buy a Core membership or $200 off when you buy an Ultra membership. Offer is only eligible on new Core and Ultra memberships and add-on specialty testing purchased together between May 22, 2026 at 12:00 AM Pacific Time and May 31st, 2026 at 11:59 PM Pacific Time. Offer valid for new members only; individuals who have previously held a membership are not eligible. Discount applies to the combined purchase of a new membership and add-on specialty testing purchased at the same time, before applicable taxes and fees; only one discount applies per transaction. Discount has no cash value and applies once per eligible individual, including memberships purchased under a family plan. This promotion cannot be combined or stacked with any other offer, code, referral incentive, credit, corporate rate, adjustment, or discount. Ultra Monthly Payment Plan Terms & Conditions: NiaHealth offers a monthly payment option for new Ultra memberships, available for both individual and family Ultra plans. Under this option, the standard individual Ultra membership of $2,199 per year is billed as 12 equal monthly payments of $183.25, and the standard family Ultra membership of $4,198 per year (for two members) is billed as 12 equal monthly payments of $349.83; members who elect monthly billing pay the equivalent of $200 more per year on an individual plan and $400 more per year on a family plan compared to paying annually upfront. Any discounts applicable at the time of enrollment are distributed evenly across all 12 monthly payments. Add-on specialty testing purchased alongside an Ultra membership under this plan is also billed monthly. If the Ultra membership is paid in full up front, then the cost is $1999 for single Ultra and $3798 for a Family Ultra membership (for 2 people). Any additional discounts for the up-front membership payment will be automatically applied at checkout. Cancellation terms: Members may cancel at any time. If a cancellation occurs within the first 6 months of membership, a $300 cancellation fee will apply to each plan member. Members can only complete their second blood draw only after 6 months of their first initial monthly payment. Upon cancellation, all future monthly charges will stop immediately and no further charges will be made. Cancellation terminates access to all remaining Ultra services not yet received, regardless of payments made to date — for example, a member who has completed a blood draw but has not yet received VO2 max testing, DEXA, or other remaining Ultra services will lose access to those services upon cancellation. NiaHealth will not seek to recover costs for services already rendered that exceed the total payments collected at the time of cancellation. This payment option is available to new members only and does not apply to renewals, transfers, gifted memberships, or corporate plans. NiaHealth reserves the right to modify or discontinue this payment option at any time, or to decline enrollment in cases of suspected abuse or fraud.
